Richmond City Health District: Northside Survey

The Richmond City Health District would like your help in setting up a collaborative Northside Health and Wellness Initiative by participation in a survey.

  1. Why: This survey will help the Northside HWI identify the main health issues affecting Northside residents;the main barriers preventing residents from accessing health-related resources; and residents’ desires for health/wellness services and programming in their neighborhood. We will use the results to determine priority focus areas for the initiative and structure its strategic plan for the coming year.
  2. What: This is a one-page (front and back) survey that should take about 15 minutes to complete. It is anonymous. Attached is a PDF so you can print them here: NS survey revised 1-29 AND/OR you can use the electronic version on survey monkey, found here: https://www.surveymonkey.com/r/TF2C9FS
  3. Deadlines: Please complete the attached surveys before March 1st.

Please also note that we are looking for a few interested residents to participate in a photovoicesurvey process, in which participants will take a few pictures around their neighborhood of things that promote health or prevent it in their daily lives, in their opinion, and share those with our steering committee.
